Understanding Brain Fog

Brain fog is not a formal diagnosis. It is a patient-friendly term for symptoms such as slow thinking, poor focus, forgetfulness, mental heaviness, difficulty processing information, or reduced clarity.

Patients may say:

Common Causes and Contributors

Sleep apnea can cause non-refreshing sleep, morning headache, daytime sleepiness, poor focus, irritability, and brain fog.

Anxiety, Stress and Burnout

What to Expect at Your First Visit

Your first visit focuses on understanding the timeline and pattern. We will ask when the brain fog started, whether it is constant or fluctuating, how sleep is, whether there is snoring or daytime sleepiness, whether mood or stress has changed, what medicines you take, and whether there are neurological symptoms.

Testing depends on the pattern. Some patients need lab testing, some need sleep evaluation, some need cognitive screening, and some need neurological testing or imaging referral.

When to Seek Medical Evaluation

You should consider evaluation if:

  • Brain fog lasts more than two to three weeks
  • Poor concentration affects work or studies
  • You feel sleepy during the day
  • You snore, choke, or wake with morning headaches
  • You have anxiety, low mood, burnout, or high stress
  • You have diabetes, thyroid disease, B12 deficiency risk, anemia risk, or chronic illness
  • Memory problems are worsening
  • Family members notice cognitive or behavior changes
  • There are neurological symptoms such as weakness, numbness, imbalance, seizures, blackouts, tremor, or confusion

How JHN Evaluates Brain Fog

Evaluation may include:

  • Timeline and pattern of symptoms
  • Sleep quality, insomnia, snoring, and daytime sleepiness review
  • Work schedule, screen exposure, caffeine, and stimulant review
  • Anxiety, depression, stress, and burnout screening
  • Medication and sedative review
  • Medical review for diabetes, thyroid disease, B12 deficiency, anemia, vitamin D deficiency, kidney/liver issues, and other contributors
  • Neurological examination when indicated
  • Cognitive screening if memory impairment is a concern
  • Sleep study / PSG if sleep apnea is suspected
  • EEG if spells, blackouts, seizure-like episodes, or episodic confusion are present
  • MRI/CT referral when neurological red flags are present

EEG means electroencephalogram, a brain-wave test. MRI/CT are imaging tests used only when the clinical pattern suggests they are needed.

Treatment Approach

Treatment may include:

  • Improving sleep schedule and sleep quality
  • Treating insomnia or sleep apnea when present
  • Correcting B12, thyroid, anemia, diabetes, vitamin D, or other medical contributors when identified
  • Medication review and reduction of sedating contributors when appropriate
  • Stress, anxiety, or mood treatment
  • Headache or migraine management when contributing
  • Work-routine and screen-habit correction
  • Cognitive and routine-structure strategies
  • Follow-up to track concentration, energy, and daily function
